Stand Firm When Seeking LGBTQ+ Affirming Care

Access to quality, respectful healthcare is a fundamental human right. Yet for many people in the LGBTQIA2S+ community (referred to here as LGBTQ+), healthcare settings can feel unsafe, invalidating, or even hostile. Discrimination, misgendering, dismissed symptoms, and outright denial of care remain real experiences for many patients. Standing firm for your right to LGBTQ+ affirming healthcare isn’t only an act of self-advocacy—it’s essential to improving health outcomes, preserving dignity, and advancing health equity.

What Is LGBTQ+ Affirming Healthcare?

This approach goes beyond treating illness. It refers to clinical environments where providers:

  • Are educated about LGBTQ+ health needs.
  • Use inclusive and respectful language.
  • Deliver care that honors sexual orientation, gender identity, and lived experience.

Affirming care also means using correct names and pronouns, collecting accurate demographic information when appropriate, and never dismissing pain, symptoms, or concerns because of bias or assumptions. Providers who embrace this methodology also participate in ongoing training and adapt health systems to meet their patients’ diverse needs.

Studies consistently show that affirming healthcare environments are linked to:

  • Better preventive care.
  • Stronger patient–provider relationships.
  • Improved management of chronic and mental health conditions.

However, despite these benefits, national data indicate that only about one-third of U.S. primary care practices offer training specific to LGBTQ+ inclusion, underscoring how much progress remains.

Why Does Affirming Care Matter for Better Health Outcomes?

Negative healthcare experiences can have lasting consequences. Many LGBTQ+ individuals report delaying or avoiding medical care altogether due to fear of discrimination or prior mistreatment. This avoidance contributes to poorer health outcomes, including untreated mental health conditions, alcohol use disorder (AUD), and substance use disorder (SUD).

Affirmation is particularly essential to older LGBTQ+ adults, who may face compounded health risks due to lifelong stigma and discrimination.

According to the Substance Abuse and Mental Health Services Administration (SAMHSA), sexual and gender minorities experience higher rates of AUDs and SUDs than heterosexual populations and often enter treatment with more severe symptoms. One key driver is minority stress—a concept defined by the American Psychological Association as the chronic psychological and physiological strain caused by stigma, discrimination, and social exclusion.

Affirming healthcare helps disrupt this cycle. When patients feel safe, respected, and understood, they are more likely to seek preventive screenings, engage in treatment early, and remain connected to care. Trust isn’t incidental to healing—it’s foundational.

Is Gender-Affirming Care Medically Necessary?

Yes, supportive healthcare environments are especially critical for transgender and gender-diverse individuals. Gender-affirming care includes medical, behavioral, and sometimes surgical services that help align a person’s physical characteristics with their gender identity. Major medical organizations—including the American Medical Association and the American Academy of Pediatrics—recognize gender-affirming care as evidence-based and medically necessary.

Multiple studies from these and other organizations show that access to gender-affirming care is associated with reduced rates of depression, anxiety, and suicidality. By contrast, denial of such care exacerbates distress and increases health risks. Standing firm in seeking affirming providers can be lifesaving.

Knowing Your Rights in Healthcare Settings

In the United States, civil rights laws provide important protections for LGBTQ+ people seeking healthcare. Section 1557 of the Affordable Care Act prohibits discrimination in health programs or activities that receive federal funding. Federal agencies interpret discrimination “on the basis of sex” to include sexual orientation and gender identity, making it illegal for covered providers to deny care for these reasons.

As of winter 2026, these protections apply broadly across healthcare settings and insurance plans. Individuals who believe they have experienced discrimination can file a complaint with the U.S. Department of Health and Human Services’ Office for Civil Rights, which investigates violations and enforces nondiscrimination laws.

What Does LGBTQ+ Affirming Care Look Like in Practice?

Prominent health systems and academic medical centers are increasingly adopting such practices to ensure LGBTQ+ patients receive high-quality, compassionate care. Here’s what you can expect:

  • Inclusive patient forms and electronic health records that allow for recognition of name, pronouns, and identity.
  • Providers asking patients about their sexual orientation and gender identity in a respectful way when relevant to care.
  • Mental health and reproductive health services tailored to LGBT+ experiences.
  • Culturally competent HIV prevention, testing, and care services.
  • Use of gender-inclusive bathrooms and waiting areas where possible.
